refactor: merge setup.sh into update.sh for seamless first-run

- update.sh now auto-generates .env from .env.example if missing
- Generates random JWT/SALT keys on first run
- Existing .env is never overwritten
- Deleted redundant setup.sh
- Updated all documentation references
